8:00 a.m. - 4:00 p.m. Monday-Friday (closed all legal holidays) / Registered Nurse On-Call 24 Hours a Day, 7 Days a Week
 
Coverage Area
All of Jackson County and into bordering counties; Wood, Kanawha, Putnam, Roane, Wirt, and Mason
 
Who Pays?
We bill Medicare, Medicaid, Veteran's Administration, Insurance, and Private Pay
Who is Eligible?
 
Individuals who are homebound and under the care of a licensed physician and in need of service.  Jackson County Health Department does not discriminate on the basis of sex, religion, race, handicap, national origin, or age.
 
Homebound Definition (Medicare/Medicaid)
Confined to home. If you have a condition which makes it difficult for you to leave your home, or leaving your home is a medically difficult and taxing effort. May leave home for infrequent periods of short duration.
 
How is Referral Made?
Hospital - Social Services, Rehabilitation Facility, Physician or Family Call

Private Duty Services (private pay)

Registered Nurse

  • Venipuncture
  • Patient/Family Teaching
  • Wound Care
  • IV Antibiotic and Hydration Therapy
  • Medication Supervision
  • Injections
  • Catheter Maintenance

Home Health Aide/Chore Worker (one hour minimum required)

  • Personal Care/Bathing
  • Light Housekeeping
  • Run Errands
  • Relieve Family / Caregiver to Run Errands or Rest
  • Meal Preparation

 

Our Staff is Well-Screened, Well-Trained, CPR Certified, and meets Yearly In-Service Training Requirements
